Elevation Men's Clinic (operated by Brazell & Associates, Inc.) is a healthcare provider and is required by the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"), the HITECH Act, and applicable Georgia law to maintain the privacy and security of your Protected Health Information ("PHI"). This section describes how we comply with those requirements.
How We Use and Disclose PHI
We use and disclose PHI only for:
Treatment — coordinating your care among our providers, pharmacies (including our compounding pharmacy partners), and any other healthcare provider involved in your treatment.
Payment — billing and processing payment for the services you receive.
Healthcare Operations — quality improvement, staff training, and normal business operations of the clinic.
We will not use or disclose your PHI for any other purpose, including marketing, without your written authorization, except as required or permitted by law (for example, in response to a court order, public health reporting requirement, or to avert a serious threat to health or safety).
Business Associates
Where we share PHI with third parties who perform services on our behalf (such as our EMR provider, CRM/scheduling platform, or compounding pharmacies), we maintain Business Associate Agreements ("BAAs") requiring those parties to safeguard your information consistent with HIPAA.
Your Rights Regarding Your PHI
Under HIPAA, you have the right to:
Request access to and receive a copy of your medical and billing records
Request that we amend your health information if you believe it is incomplete or incorrect
Request an accounting of certain disclosures of your PHI
Request restrictions on certain uses or disclosures of your PHI
Request confidential communications (e.g., that we contact you at a specific phone number or address)
Receive a paper copy of this notice upon request
Be notified in the event of a breach of your unsecured PHI
File a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights, if you believe your privacy rights have been violated, without fear of retaliation

Data Security
We maintain administrative, technical, and physical safeguards appropriate to the sensitivity of PHI in our systems, including our electronic medical record and CRM platforms, consistent with the HIPAA Security Rule.
Georgia Patients
Elevation Men's Clinic provides services exclusively to patients located in the state of Georgia and maintains medical records in accordance with Georgia law governing the confidentiality and retention of patient health records (O.C.G.A. § 31-33-2 et seq.).
Changes to This Notice
We reserve the right to change the terms of this notice and to make the revised notice effective for PHI we already have as well as any information we receive in the future. The current version will always be posted on this page, with the effective date noted below.

Identifying Purposes
